Resources and Support

During and after your stay at our hospital, appropriate resources will be made available to provide care and support to you, your family members or caregivers; answer questions; and advance your recovery. Your rehabilitation physician, for instance, will visit you to attend to your medical conditions and address any concerns you may have, as will the other members of your clinical team. Your case manager will visit, coordinate your care, and provide a phone number to answer any of your questions. Additionally, our staff will arrange family or caregiver training as needed before you leave to go home.

In addition to hospital staff and its community support groups, you will find a number of online national resources that can be helpful with your recovery and answer any questions you might have after you leave the hospital.

Rehab Support Groups

Community Rehabilitation Hospitals offer free monthly support groups for patients, families and caregivers dealing with brain injury, stroke and amputation. Support groups encourage sharing of common concerns and goals in an atmosphere of camaraderie and friendship. Support groups take place at Community Rehabilitation Hospital North.

Brain Injury Support Groups

Who: Anyone recovering from a brain injury who would like to share experiences or difficulties.
When: Third Tuesday of each month from 2 to 3 p.m.
Where: Community Rehab Hospital North, First Floor Day Room

Stroke Support Group

Who: Anyone recovering from a stroke who would like to share experiences or difficulties.
When: Third Tuesday of each month from 2 to 3 p.m.
Where: Community Rehab Hospital North, First Floor Day Room

Amputee Support Group

Groups are held in partnership with Advanced OrthoPro, Inc. (AOI).

When: Second Monday of each month from 4 to 5 p.m.

Peer Visitor Program

Community Rehabilitation Hospital North has an outreach program to offer support and encouragement to stroke survivors and family caregivers. The Stroke Connection Peer Visitor program, sponsored by the American Heart Association, is designed to match recovering stroke survivors and caregivers with persons or families who have been recently affected by stroke. We also offer peer visitor programs for patients with spinal cord injuries and amputees.

Peer visitors are specially chosen. They participate in training with a senior peer visitor, during which they learn how to provide visitor support services. Through weekly personal visits, peer visitors meet with patients and family caregivers to provide education, wellness information, advice and support during the difficult periods of recovery.
